This course outlines simple steps to help you reach your healthier YOU goals. A special emphasis is placed on weight loss and disease prevention.

Course Description

This online course was developed by the Southwest Utah Public Health Department and is for anyone seeking a healthier lifestyle. The course is designed to be completed at your own pace. Each step takes about 15 minutes to complete.

How it works

This course outlines 10 simple steps to help you reach your healthier YOU goals. The steps are accompanied by short readings, videos, and action steps. In addition, the course offers participants support from the course instructor and a registered dietitian to offer guidance and encouragement. 

As an incentive, after completing the course you will be eligible for a free A1C test or a home blood pressure monitor. 

The A1C test involves a simple finger prick. You will get your results within a few minutes, placing you in the range of normal, prediabetic, or diabetic. 

To receive a free A1C test or home blood pressure monitor you must be at least 18 years old and a resident of Washington, Iron, Kane, Garfield, or Beaver County.

About the Instructor

Jeff Smith has over 20 years of experience in healthcare and adult education.  In addition, he has a background in web media, program improvement, and community outreach.

Jeff has 3 immediate family members with diabetes and has battled with making healthy choices and regular physical activity himself. He has seen the positive and rewarding impact good health education can have on individuals, families, and communities.

Jeff is currently a Project Manager at the Southwest Utah Public Health Department where he works to improve health outcomes for the residents of Washington, Iron, Kane, Garfield, and Beaver Counties.

Prior to his current position at the Public Health Department, Jeff served as the lead Quality Improvement Consultant for the Everyone with Diabetes Counts Program at Qualis Health that received national recognition. Jeff also served as the Quality Assurance Supervisor for a statewide Medicaid contract. Jeff has taught Basic Skills Education Courses at Boise State University and the College of Western Idaho. 

He earned a Master of Public Administration Degree and a Certificate in Gerontology from California State University, Long Beach and a Web Media Certificate from Brigham Young University - Idaho. Jeff is a Certified Lifestyle Coach from the University of Utah and a Certified Diabetes Self-Management Program Master Trainer from Stanford University.
